Delete Your Account

Persimmon is committed to protecting your privacy and safeguarding personal information in accordance with applicable federal and state laws. This policy explains how Providers may request deletion of their Persimmon Providers account and how Persimmon manages personal information and records following an approved deletion request.

Requesting Account Deletion

You may request deletion of your Persimmon Providers account at any time by:

Email

Send an email to help@persimmon.life with the subject line "Account Deletion Request."

Include:

  1. The email address associated with your account

  2. Your full legal name

  3. Any additional information reasonably requested to verify your identity

You may also initiate an account deletion request within the Persimmon Providers application by navigating to:

Profile → Delete My Account

To protect your privacy and security, Persimmon may require identity verification before processing your request.

Processing Your Request

Following verification of your identity, Persimmon will review your request and process it in accordance with applicable federal and state law.

Depending on your relationship with Persimmon, account deletion may include:

  1. Closure of your user account

  2. Removal of access to the Providers platform

  3. Deletion or de-identification of personal information that Persimmon is not legally required to retain

  4. Revocation of future access credentials

Deletion requests are subject to applicable legal, regulatory, contractual, and patient safety obligations.

Information That May Be Deleted

Where permitted by law, Persimmon may delete or permanently de-identify information such as:

  1. User profile information

    • Name

    • Email address

    • Telephone number

    • Mailing address

  2. Login credentials

  3. User preferences and settings

  4. Marketing preferences

  5. Uploaded profile photographs

  6. Non-clinical communications not required to be retained

  7. Device identifiers associated solely with your account

  8. Analytics and usage information that can be deleted or de-identified

Certain information may instead be permanently de-identified so it can no longer reasonably be associated with you.

Information That Must Be Retained

Federal and state laws require healthcare organizations to retain certain records, even after an account has been deleted.

Accordingly, Persimmon may retain information including, but not limited to:

Medical and Clinical Records

Records relating to patient care, including:

  1. Medical documentation

  2. Treatment records

  3. Telehealth documentation

  4. Clinical notes

  5. Prescriptions

  6. Orders

  7. Informed consent documentation

  8. Images that form part of the medical record

  9. Communications incorporated into the medical record

These records are retained for the periods required by applicable federal, state, and professional licensing requirements.

Provider Credentialing Records

Including:

  1. Professional licenses

  2. DEA registrations

  3. Certifications

  4. Credentialing documentation

  5. Privileging records

  6. Background screening documentation

  7. Continuing education records

  8. Compliance documentation

Financial and Business Records

Including:

  1. Billing records

  2. Payment history

  3. Tax documentation

  4. Accounting records

  5. Invoices

  6. Transaction history

  7. Refund records

These records may be retained to comply with tax, accounting, and financial reporting obligations.

Compliance and Regulatory Records

Including:

  1. Audit logs

  2. Security logs

  3. Access logs

  4. Incident investigations

  5. Fraud prevention records

  6. HIPAA compliance documentation

  7. Regulatory reporting records

  8. Quality assurance documentation

  9. Risk management documentation

Legal Records

Persimmon may retain information necessary to:

  1. Comply with applicable law

  2. Respond to governmental requests

  3. Defend legal claims

  4. Resolve disputes

  5. Enforce contractual obligations

  6. Protect the safety and security of patients, providers, and the platform

Backup Systems

Information contained in encrypted backups may remain until those backups are securely overwritten or destroyed according to Persimmon's disaster recovery, business continuity, and information security practices.

Backup information is not actively used except for authorized recovery, security, legal, or regulatory purposes.

Medical Record Retention

If information associated with your account forms part of a patient's designated medical record or another legally required healthcare record, Persimmon will retain those records for the period required by applicable federal and state law and professional licensing requirements.

Account deletion does not require deletion of medical records or other records that healthcare providers are legally obligated to maintain.

Your Privacy Rights

Depending on your state of residence, you may have certain rights regarding your personal information, including the right to request:

  1. Access to personal information

  2. Correction of inaccurate personal information

  3. Deletion of personal information, subject to applicable legal exceptions

  4. Information regarding Persimmon's processing of personal information

Certain requests may be denied or limited where permitted or required by law, including circumstances involving healthcare records, fraud prevention, security, regulatory compliance, contractual obligations, patient safety, or other legal requirements.

Identity Verification

To protect your personal information, Persimmon will verify your identity before fulfilling requests to delete, access, or correct personal information.

If Persimmon cannot reasonably verify your identity, your request may be denied until sufficient verification is provided.
